About MCSA-5876 2024 Form
MCSA-5876 is a form used by the Federal Motor Carrier Safety Administration (FMCSA) to verify a commercial motor vehicle (CMV) driver's medical fitness for duty. This form is specifically designed for drivers to report any medical conditions that may affect their ability to operate a CMV safely. The MCSA-5876 form is required for all CMV drivers who are subject to the FMCSA's medical examination requirements. This includes drivers who operate vehicles that have a gross vehicle weight rating (GVWR) or gross combination weight rating (GCWR) of over 10,000 pounds, vehicles that are designed to transport more than 8 passengers for compensation, and vehicles that transport hazardous materials in quantities that require placarding. Furthermore, drivers must complete this form each time they undergo a physical examination to obtain or renew their Commercial Driver's License (CDL). This ensures that the driver's health and medical history are up-to-date and that they meet the FMCSA's medical qualification standards.
